Pick up your contest form at our office or download it from our social media. Please complete and take photos of each activity (because if there isn’t a pic it didn’t happen)!

Once your form is completed, you can email it with your name, phone number and all the photos to marketing@smileok.com or drop off at any of our locations.

Remember: all forms must be submitted by June 30th!

All participants who submit entries will be entered into a random drawing for a chance to win two season passes to Frontier City and Hurricane Harbor!

Two winners will be chosen, so get ready to have fun and show us your summer smile!

I really like this practice. They have AI assisted treatment so my daughter doesn’t have to visit the office so often. I receive updates either the... same day or next day after she does her scan. The device attaches to her phone, and she can do it all on her own.They send reminders if she forgets to do it, and there are directions for her after the scan to help her treatment.They do same day starts, which is amazing, I didn’t have to take another day off of work. Also, filling out insurances was a breeze. The payment options help so much.I definitely recommend CSO for orthodontic therapy!!!I’ll add photos to my post when I get home to my daughter!!!read more
